Overview

Create the core database schema and DocumentIndex class for the global document library system.

Details

Add to substrate/ledger.py:

document_index table - Main index with doc_id, doc_type, content, chunks, embeddings
document_sources table - Track configured sources (directories, artifact store, templates)

Create substrate/document_index.py:

DocumentIndex class with index_document(), search(), get_document()
Basic CRUD operations
Integration with existing ledger

Acceptance Criteria

Database tables created
DocumentIndex class implemented
Basic indexing works
Tests pass
