Description

Analogous to inline speaker creation. Users must currently go to Manage tab to create subjects. Add an "Add Topic" button in the subject chip row on the recording screen.

Scope

Add a trailing "+" chip at the end of the subject LazyRow in RecordingScreen
Tapping it opens an inline dialog for subject name input
Support optional parent subject selection (hierarchical subjects already exist in the data model)
After creating the subject, auto-select it (call setCurrentSubject(newId))
Extract AddSubjectDialog to be reusable (currently private to MainScreen.kt)

Acceptance Criteria

"+" chip appears at the end of the subject chip row during recording
Tapping it opens a name input dialog with optional parent subject dropdown
After creation, the new subject is auto-selected as the current subject
Dialog is the same component used in ManageScreen (no duplication)
