No rate limiting on token generation endpoint.
Impact
Vulnerable to abuse or accidental DoS.
Suggested Fix
Add configurable rate limiting per client/IP.
Closed by pipeline sweep — no code changes required (blocked:no-changes). Fixer now auto-closes this case going forward.
